The

of equipment needed and its availability.

When the equipment becomes available, you will be sent an appointment
letter. If the scheduled time and date is inconvenient for you
to come to Providence, contact our office to reschedule. You
must bring a photo identification such as a Drivers License,
Student ID, Department of Elderly Affairs card, etc. If
you are unable to come in, a family member my come for you.
